• 1 Emergency Exam + any necessary X-rays (as needed)

• Fluoride Application for adults and children

• 15% Off all other in-house dental procedures

Note: Procedures performed by outside specialists are not covered.

 Additional Details

• This is a savings plan, not insurance — cannot be combined with other dental insurance.

• No waiting periods, annual maximums, or pre-authorizations.

• Valid only at our office.

• Periodontal patients may require extra cleanings:

  • $99 per additional cleaning (3- or 4-month recall)

  • 15% discount on deep cleanings (scaling and root planing)

• Membership is prepaid for 12 months and starts the day you enroll.
